Referral to Subspecialists – Sugarloaf Medical

When needed, Sugarloaf Medical will refer patients to other subspecialists.
If we refer you to a subspecialist, it is your responsibility to make the appointment with the subspecialist. If you are unable to make an appointment with subspecialist, please inform Sugarloaf Medical personnel to help facilitate setting up an appointment. If you prefer, you may choose your own subspecialist.
Sugarloaf Medical asks that you please inform us if your health insurance or plan requires prior authorization for a referral. We are not responsible for ensuring that any referral to a subspecialist, physician, or practice is covered by your particular insurance policy.
